Dr. Tammy Hart is a Clinical Assistant Professor in the Division of Clinical & Administrative Sciences. Dr. Hart currently serves as Clinical Coordinator of Disease State Management for the Jefferson Community HealthCare Clinics. Through this partnership, she maintains an ambulatory care clinical practice site for the fourth-year pharmacy students and third year experiential program. Additionally, she serves as a residency preceptor for both the community and pharmacy practice residencies program at Xavier. Her practice focuses on a team based approach in ambulatory care with an emphasis in diabetes, hypertension, hyperlipidemia, and obesity. She is a Licensed Pharmacist registered in the state of Louisiana.
Dr. Hart earned both a Bachelor of Science degree in pharmacy and a post-baccalaureate Doctor of Pharmacy degree from Xavier University of Louisiana. Prior to joining the faculty at Xavier, she was a practicing hospital pharmacist at Touro Infirmary and clinical pharmacist with Excelth, Inc. Health Clinics within the New Orleans area.
Dr. Hart currently teaches Renal Nutrition in the Basic and Clinical Nutrition course and has contributed to the Nephrology series within the Disease State Management course. She also coordinates the Pharmacy Seminar course.
Dr. Hart is currently a member of the American Society of Health-System Pharmacists (ASHP), the American Association of Diabetes Educator (AADE), and the American Diabetes Association (ADA).
