The College of Pharmacy Academic Standing Committee will review requests for readmission by former pharmacy students. A student who left the university in good academic standing and wishes to return should submit an Application for Readmission directly to the Coordinator of Student Records and Academic Support in the College of Pharmacy.
A student who has not been enrolled due to academic reasons (dismissal, probation, etc.) must complete: 1) an Application for Readmission; 2) a Request for Readmission Form; and 3) a Probationary Action Plan. The Probationary Action Plan will be due prior to the first day of class during the semester in which the student re-enrolls. These documents should be submitted to the Office of Student Affairs in the College of Pharmacy.
A student who was not in good academic standing (cumulative G.P.A. below 2.0) when he or she left the College of Pharmacy must schedule an appointment with the Counseling Center after an application for readmission has been submitted. If a student is denied readmission by the College of Pharmacy Academic Standing Committee, the student has a right to a written appeal of the decision to the University Academic Standing Committee. This appeal (University Academic Appeal Form) must be received in the Registrar’s Office by July 1.
