Dr. Hart provides didactic instruction in areas such as the history of pharmacy, integral aspects of professional pharmacy practice, self-care topics (diabetes, dermatology, smoking cessation) and endocrine disorders (diabetes mellitus, thyroid, and adrenal) in the PharmD curriculum and also maintains an active clinical practice site in a Patient-Centered Medical Home (PCMH) and Federally Qualified Health Center (FQHC), which services are to the impoverished, uninsured, and underinsured communities. Her area of focus in this ambulatory care setting is to deliver comprehensive, disease management utilizing a chronic care model approach.
As a clinical pharmacist, Dr. Hart has served the Greater New Orleans and neighboring communities for over twenty-five years. She is dedicated to educating future pharmacists, and commitment to promoting the elimination of health disparities of chronic diseases that plague our nation. Dr. Hart currently serves as the clinical care coordinator of pharmacy services at CareSouth Medical and Dental, Inc. located in Baton Rouge, Louisiana. Through this collaborative partnership, she offers health improvement, wellness, and/or disease prevention sessions that are designed to engage medical staff and patients regarding diabetes, hypertension, hyperlipidemia, and obesity. Patient outcomes are measured routinely through direct follow up visits to evaluate progress and redesign regimens as needed. Additionally, as a clinical assistant professor and preceptor, Dr. Hart effectively provides educational and experiential experiences to students enrolled in the pharmacy program.
Dr. Hart earned both a Bachelor of Science degree in pharmacy and a post-baccalaureate Doctor of Pharmacy degree from Xavier University of Louisiana. Prior to joining the faculty at Xavier, she was a practicing hospital pharmacist at Touro Infirmary and clinical pharmacist with Excelth, Inc. Health Clinics within the New Orleans area.
